Moderation queue anomalies

For some reason HelpRoom questions still show up in the moderation queue, hasn't that been changed?
People also seem to be able to see unmoderated questions from default and answer them. I doubt they do it in the queue and don't publish or reject them.
Feels stupid to publish a question that's already at its end.
